Suicides and aggression

Among indicators of adequacy of therapy are satisfaction of patients and their relatives, return of patients to normal life, lack of suicides and aggression associated with unsatisfactory mental condition, as well as lack of escapes from hospitals. Although the latter is also accounted for by the availability of a reliable security service. A suicide in a psychiatric hospital where patients are monitored round-the-clock — is an extraordinary event, negligence of practitioners and personnel, and an indicator of ineffective therapy.

Cases of suicides or suicidal attempts were encountered in hospitals rather rarely. One of the exceptions was the Republican psychiatric clinic of Bashkortostan where four suicides occurred in 2002. Two suicides occurred in each of the Tver regional psychiatric clinics, #1 and #2, as well as in the Shadrinsk neuro-psychiatric dispensary of the Kurgan region. One suicide occurred in each of the following institutions: the Astrakhan regional psychiatric clinic, the Arkhangelsk regional psychiatric clinic #1, the Regional psychiatric clinic of the Jewish autonomous district, the Krasnodar territorial psychiatric clinic, the Moscow psychiatric clinic #13, the Novgorod regional psychiatric clinic, the Penza regional psychiatric clinic named after Yevgrafov, the Kuznetsk city psychiatric clinic of the Penza region, the Primorsky territorial psychiatric clinic #1, the Republican neuro-psychiatric dispensary of Adygea, the Troitsk psychiatric clinic of the Republic of Buryatia, the Sverdlovsk regional psychiatric clinic #1, and the Khabarovsk territorial psychiatric clinic (a total of 13 institutions). The number of suicidal attempts was significantly larger but one managed to prevent them from happening. For example, the Vladivostok city psychiatric clinic of the Primorsky territory and the Sverdlovsk regional psychiatric clinic #1 reported 15 suicidal attempts that had occurred in the previous year, the Primorsky territorial psychiatric clinic #1 reported 10 attempts, the Republican psychiatric clinic of Tatarstan — five, the Astrakhan regional psychiatric clinic, Republican neuro-psychiatric dispensary of Buryatia and the Republican psychiatric clinic of Komi — four, the Arkhangelsk regional psychiatric clinic #1 and the Smolensk regional psychiatric clinic — three suicidal attempts.

Aggression of patients with respect to each other, as well as to personnel is rather common. (See sections on Fundamental Rights and Safety).

Escape from the hospital — is an overt demonstration of one’s unwillingness to undergo therapy, at least to do it in the conditions of an institution, and indicates both ineffectiveness of the therapy used and insufficiency of observation and security measures required in such situations.

Each hospital reported an average of several cases per year when patients leave institution’s premises of their own accord. However, if they had been institutionalized voluntarily such abandon is not viewed as escape and the patient is simply discharged. If the patient who left the hospital had been institutionalized against their will and especially one who had been in therapy in compliance with a court decision due to prior delinquent behavior, it is viewed as escape and the clinic instigates a search. Information about the escapee is forwarded to the local neuro-psychiatric dispensary and law-enforcement authorities. As a rule the patient is returned to the institution. 41 involuntarily hospitalized patients escaped in 2002 from the Perm city psychiatric clinic, 35 — from the Republican neuro-psychiatric dispensary of Kabardino-Balkaria, 30 — from the Moscow regional psychiatric clinic #2, 18 — from the Novgorod regional psychiatric clinic, 16 — from the Republican psychiatric clinic of Tatarstan, 15 — from the Sverdlovsk regional psychiatric clinic #1, 14 — from the Nizhnii Novgorod regional psychiatric clinic #1, 12 — from the Irkutsk regional psychiatric clinic #1, 10 — from the Moscow psychiatric clinic #13, and nine — from the Khabarovsk territorial psychiatric clinic. Practically all psychiatric institutions, except for those that admit patients exclusively on the voluntary basis (for example, the St. Petersburg psychiatric clinic #7 named after Pavlov) have three-four escapes per year.
