English Language Page THE RIGHT TO DECENT ENVIRONMENT
“Therapy by environment” is one of the traditional and fundamental factors of the treatment and rehabilitation process. The architecture of psychiatric institutions and internal setup of departments affect organization of the living space in which patients of psychiatric facilities reside. Design of the buildings, departments, and all the necessary facilities accounted for by the building type determine the psychological comfort of the environment — a place in which some patients spend significant periods of time.
The effectiveness of the therapeutic and rehabilitation process is higher in the conditions facilitating social activity of patients. The internal organization of psychiatric institutions must provide for the maximum possible degree of free behavior of patients accounting for peculiarities of their various categories and stimulate interpersonal interactions, as it prevents patients from loss of adaptation and simplifies their return to normal life. It benefits self-esteem and reduces the level of self-aggression. The lay-out of departments must provide for opportunities of having privacy because for the recovery of certain categories of mental patients privacy is of principal importance.
The majority of psychiatric institutions in Russia were constructed during the soviet period when primary attention in the course of construction was paid to the assurance of isolation and opportunities to observe patients directly. This was largely the result of ideas that were prevalent during that period about the desired organization of psychiatric care and specifics of an effective therapy process. Other aspects necessary for successful rehabilitation were not taken into account and opportunities for patients’ privacy were openly eradicated.
Today these principles are considered erroneous. Nevertheless the situation is not changing and the living space of patients that carries an independent therapeutic importance is organized precisely in compliance with isolation and observation priorities.
When assessing the hospital living conditions one must take into account not only structural characteristics of hospitals that set the limits to the freedom of patients, but also such important parameters as designation of buildings at the time of construction, the condition of communal and transportation facilities of the institution, as well as peculiarities of the natural environment. |
